Hangman if I may. you are 2 steps away from a vacuum form machine. all that you need now is a box use the plywood you have, drill a hole in the side for a vacuum hose and drill the working surface with 1/8 inch hole. you have a great frame and heat source, also the mold should be as smooth as possible even the heat and smash method will pick up imperfections in the mold. with the machine you won't have to use that template with the hole in it. also you may get by with those bulldog clamps similar to a clipboards clamp instead of having to bolt each of them down.
