As for more on the form, what about the suggestion for paper size, proper scaling, number of days to make the model, that's my two cents

Number of hours might be better than number of days...

Another thing to note might be difficult portions to fit together (even if they went together)

Well Number of hours for simple - middle classed models. Probably days for advanced ones...

Oh, you're right! That way the designers could try and simplify that, great thought!

As someone who has beta'd for several designers some of the feedback I give consists of

1) Parts fit/former fit
2) Panel line alignment
3) Markings/ camoflage alignment
4) Missing/Mislabeled part numbers
5) Instruction sheet advice
6) Overall impression
7) Overall shape (does it look right)

Just some ideas to throw out there.

Well, it should be kept to just one. For me, doing a beta build is a constant
communication process with the designer. Most of the time, I make my recommendation.
then the designer incorporates the suggestion, and sends a modified part back
for inclusion in the model or instruction sheet. And so it goes until done.

Sometimes. Others, this form would be handy. I'm not sure how I would incorporate
all the feedback I give. This might be a good form for the end of the build,
sort of as a guide to help establish the targeted modeller.
