I can see where the "Yellow" name comes from. According to the Wiki article, the "Doctor" is because they carry special diagnostic equipment to monitor the condition of the track and catenary. The train represented by this model is fairly new, but according to the Wiki, "Doctor Yellow" (do-ku-ta-i-ye-ro in Japanese) trains in various configurations have operated in Japan since the early 60s, but I do not recall ever seeing or riding on one. https://en.wikipedia.org/wiki/Doctor_Yellow
