I also use Preview on Mac for reorganizing and compiling PDF pages. CorelDRAW on Windows can also export to a multi-page PDF. Another tool I use is ghostscript, which I find will produce PDFs that are more "standard" and compatible with more viewers than what Corel or Preview might generate. It's command-line though, so not as easy to use.

Mac users can also prepare their own tool for editing PDFs ( and many other formats ) using a system app called Automator. With this app you can create sets of actions ( like in Photoshop ) and save them as applications or script like workflows.

Ghostscript is of course open source and (acknowledged or not) is the engine behind most (if not all) Windows based pdf printer emulaters.
