I use PDFill to create PDF files and to extract components from existing PDF files. I have been very happy with it.
It is free to use and works on all open PDF files.

Try PDFTK Builder. Very small but usefull. You can also split a multi-page pdf into individual pages with PDFTK Builder.
PDFTK Builder and other PDF Resources

If you decide to use PDFTK Builder keep in mind in many cases that if a pdf file is protected in some way you will not be able to combine it with another pdf file(s). I have run across situations where the parts and the instructions are two different pdf files but each was small enough to make a small, single pdf file when combined but would not combine into a single pdf file because of some sort of protection. You have to find a work around in order to include a protected pdf file in the single, combined file you are trying to create. There should be no problem if you are combining multiple pdf files that you have created.

Since MultiPDF Tools changed licence, I've used Apache PDFBox to join, split and encrypt PDF files. i run Linux, but since PDFBox is a Java library, it should be multiplatform. It's open source.

In the unlikely case that you're using a Linux system, there's always PDFMod (https://wiki.gnome.org/Apps/PdfMod). Nothing easier.

I've been using PDFTK Builder successfully for around 8 years now. Pretty sure it came from a cover disc of a long defunct magazine. Never noticed it update or needed it to. There are other useful tools in the 'Tool Kit' too, but the builder is by far the most used.

I mostly use The Gimp for messing around with files and formats, but so far I've not found a way to use it to build up a multi-page .pdf. from loose 1 page ones as PDFTK does so easily, with just a few click and drags. So PDFTK is still sitting permanently on my desktop where I know where to find it.

For people like me who have a Macbook, it's even easier. Open one page in Preview, drag and drop the other pages you want to join on the opened thumbnail sidebar. Save as [new name] and voilą, there is your joined PDF.
