I've had similar issues with commissions. I usually arrange in the contract that either I'll deliver it myself (which is usually driving across town or similar, but it could involve a plane flight in the worst scenarios) or I make sure they have someone to pick it up. And of course that goes into the price.
The crating and transportation is often the most complex or nearly the most complex part of a job.
