Honestly, I do not know. He did not say. I think that would be up to the person that had it and was able to get direct permission (or not) from Shunichi Makino.
Although I would have to say, do not hold your breath. (Even though he seems like a nice enough guy) I do not think that he would give permission for such (just my personal opinion).

So how many Lenshead models are there? I know of four, is that all of them?

I do have three of the original four. I am only missing the Charcoal version of the LensHead type B.
I should have downloaded it when I had the chance.
